WCM-Q Family Card

Service description

Through this service, WCM-Q employees can submit a request for a WCM-Q Family Card issued by WCM-Q for their relatives (spouse and children, above 16 years old). This card will allow the appointed relative to access certain QF facilities.

How to request

1. Due to confidentiality, we cannot ask for a copy of the required personal documents.  Please coordinate with HR and request for a "Dependents Letter" for Family Card Application through their ticketing system.
2. Download and complete the form from the attachments section of this page.
3. Obtain a digital passport size photo of your dependent with white background (jpg format).
4. Click on the REQUEST SERVICE button, complete the fields and attach the completed form and the "Dependents Letter" for Family Card Application provided by HR in the ticket.
