Service description

Through this service, the WCM-Q community can submit a request for a WCM-Q Proximity Card. Replacements and changes of access rights can also be requested via this service.                            

Depending on your request type, three separate forms can be found in the attachments section on this page:    

1. WCM-Q Proximity Card - Application Form
2. WCM-Q Proximity Card - Access Changes Form
3. WCM-Q Proximity Card - Replacement Form

Please note: If you are requesting a replacement of the proximity card due to a job title change, please attach the notification of the change from HR with the new job title details.

Service Audience

WCM-Q employees, students, affiliated faculty, alumni, volunteers, and selected contractors.

How to request

1. Download and complete the form corresponding to the service you are requesting in the attachments section of this page.
2. Obtain a digital passport size photo of yourself with white background (jpeg format); this is only applicable if this is the first time you are requesting the Proximity Card or if you would like to change the photo.
3. Click on REQUEST SERVICE button, complete the fields and attach the form and photo, if applicable.
